Open Records Request

The Sheboygan Police Department will consider Open Records Requests from citizens.  Please use form below to request specific records (not Accidents).

To Request a Copy of an Accident Report, Click Here.

Please allow 5 to 10 working days for your request to be processed
Other Reports: 25¢/page when picked up, $1.00 first page & 25¢ for each additional page when mailed or faxed.

For any costs over $5.00 you will be contacted and required to pre-pay. If locating costs exceed $50.00, the requester will pay these additional charges.

Open Records Request Online Form

Please fill out the appropriate fields for your records request. Access to the department's records is permitted to the public, as provided for in WI Stat. 19.35(1). An authorized records employee must review each record before it is released to ensure statuatory guidelines are being followed.
